What is the purpose of this trial?

This trial aims to determine if combining methotrexate with Palynziq (pegvaliase) can help individuals with PKU better tolerate their treatment and enhance its effectiveness. The trial seeks adults who either have not tried Palynziq or have used it for at least 24 weeks without dose increases but still exhibit high levels of the amino acid in their blood. Participants must be in generally good health and willing to try both medications together. Will I have to stop taking my current medications?

Participants must stop taking other PKU treatments besides Palynziq, as these are not allowed in the study.

What is the safety track record for Pegvaliase and Methotrexate?

Research has shown that pegvaliase is generally safe for humans, though it can cause some reactions. Approximately 72% of people using pegvaliase have experienced hypersensitivity reactions, which resemble allergic reactions but are less severe than anaphylaxis. Among those using pegvaliase for extended periods, 87% saw a 20% reduction in Phe levels within a year, increasing to 93.5% after two years. This trial will evaluate the combination of pegvaliase with methotrexate. Participants in the "Palynziq naïve" arm will start with methotrexate alone before adding pegvaliase, while those in the "Palynziq experienced" arm will receive both treatments concurrently. Are You a Good Fit for This Trial?

This trial is for adults aged 18-65 with phenylketonuria (PKU) who are in good health. Participants must be eligible and willing to take Palynziq and methotrexate, agree to use contraception if needed, carry two epinephrine devices during treatment, and either be new to Palynziq or have high blood levels despite ongoing treatment.

Inclusion Criteria

I am willing and able to take Palynziq and methotrexate as required by the study.
* Agree to use required contraception if they or their partner could become pregnant
I am willing to carry two epinephrine devices at all times during treatment.
