This trial is for adults (18+) diagnosed with chronic kidney disease. Participants must be able to take oral medication and meet specific criteria for certain medications based on their kidney function, diabetes status, heart failure, blood pressure, or protein in urine.

Inclusion Criteria

I have type 2 diabetes.
I am able to swallow and take pills by mouth.
I am 18 years old or older.
