Are You a Good Fit for This Trial?

The e-SINCERE Study is for patients who face economic and technological barriers to healthcare access. It's designed to help those struggling with Information and Communication Technology (ICT) challenges by providing them with stable cell phones and support.

Inclusion Criteria

Community Advisory Board Members who are health system, governmental, and community service stakeholders and able to understand English
I am over 18, completed the SINCERE screener, and understand English or Spanish.
I am over 18, completed the SINCERE screener, contacted 211 for services, and understand English or Spanish.

Exclusion Criteria

I live in a nursing facility or need help with daily activities.
I cannot speak English or Spanish.

Timeline for a Trial Participant

Screening

Participants are screened for eligibility to participate in the trial

Intervention

Participants receive either a standard referral, a study-provided cellphone, or a cellphone with digital navigation assistance to connect with community services

Follow-up

Participants are monitored for successful connection to community services and health-related quality of life
