Exercise Training for Chronic Obstructive Pulmonary Disease
(ReV UP COPD Trial)
What You Need to Know Before You Apply
What is the purpose of this trial?
This trial explores how exercise affects heart function in people with chronic obstructive pulmonary disease (COPD), a lung condition that makes breathing difficult. Researchers focus on the heart's right side and its response during rest and exercise. By identifying patterns in heart function, the study aims to improve future COPD treatments. Individuals diagnosed with COPD, over 40, and not currently experiencing a flare-up might be suitable for this trial. As an unphased trial, it offers participants the chance to contribute to groundbreaking research that could enhance COPD management.
Will I have to stop taking my current medications?
The trial does not specify if you need to stop taking your current medications. However, if you have changed your COPD therapy in the 3 weeks before joining, you may not be eligible to participate.
What prior data suggests that exercise training is safe for individuals with COPD?
Research has shown that exercise is generally safe for people with chronic obstructive pulmonary disease (COPD). Studies have found that exercise can improve lung function, increase exercise capacity, and enhance quality of life for these patients. For example, one study found that aerobic exercise helped people with COPD walk longer distances and reduced their symptoms.
Another study demonstrated that high-intensity interval training benefited lung function and overall health. Importantly, these studies did not report any major safety concerns or negative effects from exercise. This suggests that most people with COPD can handle exercise well.12345Why are researchers excited about this trial?
Exercise training is unique because it offers a non-pharmacological approach to managing Chronic Obstructive Pulmonary Disease (COPD). Unlike traditional treatments, such as bronchodilators and steroids that aim to open airways and reduce inflammation, exercise training targets the cardiovascular system, specifically improving right ventricular function. Researchers are excited about this because it could enhance heart and lung efficiency, potentially improving quality of life and physical endurance for people with COPD without the side effects associated with medications.
What evidence suggests that exercise training might be an effective treatment for COPD?
Studies have shown that exercise can greatly help people with chronic obstructive pulmonary disease (COPD). Research indicates that exercise enables individuals to engage in more physical activities without tiring easily. High-intensity interval training (HIIT) improves lung function and enhances quality of life for those with COPD. Pulmonary rehabilitation, which includes exercise, plays a crucial role in reducing symptoms like shortness of breath and increasing exercise capacity. Overall, regular exercise can ease daily tasks and enhance well-being for people with COPD. Participants in this trial will undergo exercise training to assess its impact on right ventricular dysfunction in COPD.12678

Are You a Good Fit for This Trial?
This trial is for people aged 40 or older with COPD whose lung function tests show obstruction. Participants should not have had recent changes in COPD treatment, severe heart problems, active cancer (except skin), very high oxygen needs, recent COPD flare-ups, or other serious health issues that make exercise unsafe.Inclusion Criteria
Exclusion Criteria
Timeline for a Trial Participant
Screening
Participants are screened for eligibility to participate in the trial
Baseline Testing
Participants complete cardiopulmonary exercise testing (CPET) and transthoracic echocardiography (TTE) during rest, moderate- and high-intensity exercise
Exercise Training Intervention
Participants undergo a targeted exercise training intervention based on their RV contractile response during baseline testing, performed 3x/week for 12 weeks on an upright stationary bicycle
Post-Intervention Testing
Participants repeat CPET and TTE to determine the impact of exercise training on right ventricular dysfunction and complete a health-related quality of life questionnaire
Follow-up
Participants are monitored for safety and effectiveness after the intervention, including exercise training safety and feasibility
What Are the Treatments Tested in This Trial?
Interventions
- Exercise training
Trial Overview
The study looks at how the right side of the heart responds to different levels of exercise in people with COPD. It involves supervised and possibly remote exercise training sessions to monitor heart function during rest and moderate-to-high intensity activity.
How Is the Trial Designed?

Treatment groups
Experimental Treatment
Participants will complete testing at rest and during moderate- and high-intensity exercise to identify patterns of right ventricular responses. Participants who based on baseline testing are eligible and decide to participate in the optional exercise training program will complete exercise training followed by repeat testing to determine the impact of exercise training on right ventricular dysfunction.
