High-Intensity Interval Training for Prostate Cancer
(ESCAPE Trial)
What You Need to Know Before You Apply
What is the purpose of this trial?
This trial explores how a short session of home-based high-intensity interval training (HIIT) affects hormone levels in men monitoring their low-grade prostate cancer without active treatment. Participants will complete an online HIIT workout consisting of six one-minute exercises. Men with localized prostate cancer who are not receiving treatment and can handle moderate-to-high intensity exercise may be suitable candidates. As an unphased trial, this study provides a unique opportunity to contribute to research that could enhance future prostate cancer management.
Do I have to stop taking my current medications for the trial?
The trial does not specify if you need to stop taking your current medications, but you cannot participate if you are using medications that affect steroid metabolism.
What prior data suggests that this high-intensity interval training is safe for men with low-grade prostate cancer?
Research has shown that high-intensity interval training (HIIT) is generally safe for men with prostate cancer. Studies reported no serious negative effects from HIIT. Participants exercised intensely without major safety concerns. HIIT also improved fitness and reduced prostate-specific antigen (PSA) levels, which are important for prostate health. This suggests that HIIT is not only safe but might also help manage prostate cancer symptoms.12345
Why are researchers excited about this trial?
Unlike traditional treatments for prostate cancer, which often include surgery, radiation, hormone therapy, or chemotherapy, high-intensity interval training (HIIT) exercise offers a non-invasive approach. Researchers are excited about HIIT because it can be done at home, making it accessible and convenient. This method also aims to improve physical fitness and potentially enhance quality of life without the side effects associated with more aggressive treatments. Additionally, exercise may have beneficial effects on cancer progression through mechanisms like improved immune function and hormone regulation.
What evidence suggests that high-intensity interval training is effective for prostate cancer?
Research has shown that high-intensity interval training (HIIT) can benefit men with prostate cancer. Studies indicate that HIIT improves the heart and lungs' ability to supply oxygen during exercise. It also lowers levels of prostate-specific antigen (PSA), a protein produced by the prostate that can signal cancer progression. Some studies suggest that HIIT may slow prostate cell growth and improve overall cancer outcomes. These findings offer hope for using HIIT to manage prostate cancer. Participants in this trial will engage in a home-based HIIT exercise session, supervised online, and will collect saliva samples to further explore these potential benefits.13456

Are You a Good Fit for This Trial?
This trial is for men aged 18–80 with localized prostate cancer who are being monitored (active surveillance), can move independently, and are able to safely do moderate-to-high intensity exercise. It excludes those with advanced cancer, certain health conditions, or on medications affecting hormones.Inclusion Criteria

Trial Overview
The study tests how a single session of home-based high-intensity interval training (HIIT) affects steroid hormone levels in men under active surveillance for low-grade prostate cancer.

Experimental Treatment
Participants will complete a single supervised, online high intensity interval training (HIIT) exercise session with six bouts of 1-minute exercises performed at a moderate-to-high intensity lasting approximately 20 minutes. Saliva samples will be collected immediately prior to exercise, immediately following exercise, and 15 minutes post-exercise.
