Community Health Worker Intervention for Lung Cancer Screening
(CHATS Trial)
What You Need to Know Before You Apply
What is the purpose of this trial?
The trial aims to find better ways to encourage lung cancer screening. It tests two approaches: one using community health workers and the other using trained lay navigators. Both groups assist with decision-making, tobacco treatment, and overcoming barriers to care. This trial may suit individuals aged 50 to 80 with a history of smoking who receive primary care at specific community health centers. As an unphased trial, it provides a unique opportunity to contribute to innovative strategies that could improve lung cancer screening rates.
What prior data suggests that this intervention is safe for lung cancer screening?
Research has shown that programs using community health workers and patient guides are generally well-accepted. Previous studies on similar programs, such as those with lay navigators, demonstrated their safety. These programs assist patients by guiding them through their healthcare journey and facilitating access to care. No major reports of negative effects have emerged from using community health workers or lay navigators. Participants typically find these programs helpful and unobtrusive, with no serious side effects reported. This suggests that the programs in the current trial are likely safe and well-received.12345
Why are researchers excited about this trial?
Researchers are excited about this trial because it explores innovative methods to improve lung cancer screening uptake. Unlike traditional methods, which often rely solely on educational materials, this trial tests two interventions: one delivered by community health workers and another by trained lay navigators. Both interventions offer a comprehensive approach, combining patient outreach, shared decision-making, tobacco treatment counseling, and assistance with overcoming logistical barriers. This personalized and supportive method aims to make lung cancer screening more accessible and engaging, potentially leading to higher participation rates and earlier detection of lung cancer.
What evidence suggests that this trial's interventions could be effective for improving lung cancer screening uptake?
Research shows that community health workers (CHWs) can effectively boost lung cancer screening rates. In this trial, one group of participants will receive a four-part intervention delivered by CHWs, including patient outreach, shared decision-making, tobacco treatment counseling, and navigation of logistical barriers. Studies have found that CHW programs help participants smoke fewer cigarettes, with 29% quitting completely after a year. Another study discovered that guiding patients through the healthcare system significantly increases lung cancer screening among smokers.
Another group in this trial will receive the same intervention delivered by lay navigators. Research supports the effectiveness of lay navigator programs, with a review of studies showing that these programs improve screening rates in high-risk groups. Lay navigators enhance patient care and coordination, leading to better screening results. Both approaches in this trial aim to overcome obstacles and improve access to lung cancer screening and tobacco treatment.678910Who Is on the Research Team?

Are You a Good Fit for This Trial?
This trial is for adults aged 50-80 who get their primary care at Mason Square, High Street, or Brightwood community health centers. Participants must speak English or Spanish and have a smoking history that makes them eligible for lung cancer screening.Inclusion Criteria

Timeline for a Trial Participant
Screening
Participants are screened for eligibility to participate in the trial
Intervention
Participants receive a multi-component intervention including patient outreach, shared decision-making, tobacco treatment counseling, and navigation of logistical barriers to care.
Follow-up
Participants are monitored for feasibility, fidelity, and acceptability of the intervention, as well as lung cancer screening and tobacco treatment utilization.
Long-term Follow-up
Participants' outcomes are evaluated for lung cancer screening uptake and tobacco treatment initiation.
What Are the Treatments Tested in This Trial?
Interventions
- Community health worker delivered outreach, shared decision-making, tobacco treatment, and navigation
Trial Overview
The study compares two approaches: one group gets help from community health workers offering outreach, shared decision-making about lung cancer screening, tobacco treatment support, and navigation; the other group receives enhanced usual care.

Active Control
Participants will receive the same four-part intervention delivered by a trained lay navigator to improve lung cancer screening uptake. The intervention includes: 1) patient outreach, 2) patient-centered shared decision-making, 3) tobacco treatment counseling, and 4) navigation of logistical barriers to care.
