Doxepin + Ramelteon for Sleep Disturbance
What You Need to Know Before You Apply
What is the purpose of this trial?
This trial tests whether the medications doxepin and ramelteon can improve sleep quality for those recovering from total knee replacement surgery. Poor sleep during recovery can worsen pain and slow healing. Participants will receive either doxepin alone, doxepin combined with ramelteon, or a placebo to determine which offers the best results. Individuals who have recently undergone knee replacement and are not currently using certain mental health medications or sleep aids might be suitable for this study. As a Phase 4 trial, this research involves FDA-approved treatments and aims to understand how they can benefit more patients.
Will I have to stop taking my current medications?
The trial requires that you stop taking certain medications, such as SSRIs, SNRIs, MAOIs, prescription sleep aids, and pre-operative narcotics. If you are on these medications, you may not be eligible to participate.
What is the safety track record for these treatments?
Research has shown that both doxepin and ramelteon are generally safe for improving sleep. Studies have found that doses of 1 mg, 3 mg, and 6 mg of doxepin are well-tolerated by adults with insomnia, improving sleep without major side effects. However, some rare sleep-related behaviors, such as sleepwalking, have been reported.
The combination of doxepin with ramelteon is also considered safe. Ramelteon, approved by the FDA for sleep problems, usually does not cause serious side effects. Some studies suggest that using these two together can effectively treat insomnia. While doxepin may enhance ramelteon's effects, no major safety concerns have been identified.
Overall, both doxepin and the doxepin-ramelteon combination appear to be well-tolerated and safe options for addressing sleep issues.12345Why are researchers enthusiastic about this study treatment?
Researchers are excited about the combination of Doxepin and Ramelteon for sleep disturbances, especially after knee surgery, because it brings together two distinct mechanisms of action. Doxepin, a tricyclic antidepressant, is used at a low dose to target histamine receptors, promoting sleep by reducing wakefulness. Meanwhile, Ramelteon works differently by mimicking melatonin, helping regulate the sleep-wake cycle. This dual approach could provide more comprehensive relief for sleep disturbances than current options, which often rely on either sedative hypnotics or medications that focus solely on one pathway.
What evidence suggests that this trial's treatments could be effective for sleep disturbance after total knee arthroplasty?
Research has shown that doxepin can help adults with insomnia sleep better. Studies have found that doses of 1 mg, 3 mg, and 6 mg of doxepin help people stay asleep longer and fall asleep faster. In this trial, one group of participants will receive doxepin 6 mg alone, while another group will receive a combination of doxepin 6 mg and ramelteon 8 mg. Ramelteon helps regulate the sleep-wake cycle, making it easier to fall asleep. Together, these medications have shown promise in improving sleep without causing significant side effects.12467
