Are You a Good Fit for This Trial?

This trial is for English-speaking residents in long-term care who are quite frail, with a score of 50% or less on the Palliative Performance Scale. It's not suitable for those who aren't fluent in English or are less frail (score above 50%).

Inclusion Criteria

English-speaking LTC residents with a score of 50% or less on the Palliative Performance Scale

Exclusion Criteria

Non-English speaking LTC residents
Residents with a score of more than 50% on the Palliative Performance Scale.

Screening

Participants are screened for eligibility to participate in the trial

Intervention

Implementation of the SPA-LTC program in long-term care homes across three provinces

up to 160 weeks
Weekly checklists completed

Follow-up

Participants are monitored for family satisfaction, staff knowledge, and hospital use after the intervention

1-2 months

Post-Bereavement Follow-Up

Follow-up with families and staff after bereavement

6 weeks to 2 months after resident's death
