ABBV-383 for Waldenstrom's Macroglobulinemia
What You Need to Know Before You Apply
What is the purpose of this trial?
This trial tests a new treatment called ABBV-383 for individuals with Waldenström's Macroglobulinemia, a type of blood cancer that has returned or isn't responding to treatment. ABBV-383 is a monoclonal antibody, a protein designed to help the immune system target and destroy cancer cells. The trial aims to determine the best dose, assess its effectiveness, and understand any side effects. Individuals diagnosed with relapsed or refractory Waldenström's Macroglobulinemia who have previously received treatment with a Bruton tyrosine kinase (BTK) inhibitor might be suitable candidates for this trial. As a Phase 1, Phase 2 trial, this research focuses on understanding how the treatment works in people and measuring its effectiveness in an initial, smaller group.
Do I have to stop taking my current medications for the trial?
The trial protocol does not specify if you need to stop taking your current medications. However, certain prior therapies like chemotherapy, investigational therapies, and steroid therapy close to the trial start date are not allowed. It's best to discuss your specific medications with the trial team.
Is there any evidence suggesting that ABBV-383 is likely to be safe for humans?
Research has shown that ABBV-383, a monoclonal antibody, has been safe in studies. Administering doses of 20 mg, 40 mg, and 60 mg every three weeks resulted in few side effects that led to treatment discontinuation. While some side effects occurred, they were generally not severe enough for most patients to stop treatment. As this trial is in its early stages, the primary aim is to identify any side effects and determine the optimal dose. Although safety information is still being collected, early results appear promising.12345
Why do researchers think this study treatment might be promising for Waldenstrom's Macroglobulinemia?
ABBV-383 is unique because it introduces a novel approach to treating Waldenstrom's Macroglobulinemia by targeting specific pathways that are not addressed by standard treatments like Rituximab, Ibrutinib, or chemotherapy. It is administered intravenously, allowing for direct and controlled delivery into the bloodstream, which might enhance its effectiveness and reduce side effects compared to oral medications. Researchers are excited about ABBV-383 because it has the potential to offer a more targeted treatment with fewer cycles and possibly quicker responses, bringing hope for better management of the disease with fewer complications.
What evidence suggests that ABBV-383 might be an effective treatment for Waldenström macroglobulinemia?
Research has shown that ABBV-383, a monoclonal antibody, might help treat Waldenström's macroglobulinemia, particularly when the disease returns or becomes resistant to other treatments. Monoclonal antibodies target specific molecules and assist the immune system in attacking cancer cells. Early results suggest these treatments might reduce the need for ongoing, lifelong treatment by achieving deep remissions, significantly reducing or eliminating disease symptoms. Studies indicate that targeted therapies like ABBV-383 can slow or stop cancer cell growth and spread, offering hope for better outcomes in challenging cases. Participants in this trial will receive ABBV-383 as part of the treatment regimen.13467

Are You a Good Fit for This Trial?
This trial is for adults (18+) with Waldenström macroglobulinemia that has come back or not responded to treatment, who have already tried a BTK inhibitor unless they couldn't for medical reasons. Participants must be in generally stable health, able to give blood and bone marrow samples, and willing to use birth control if needed.Inclusion Criteria

Treatment
Patients receive ABBV-383 IV on days 1 and 4 of cycle 1 and on day 1 of subsequent cycles. Cycles repeat every 28 days for up to 12 cycles.
Follow-up
Participants are monitored for safety and effectiveness after treatment, including CT or PET/CT scans and blood and urine sample collection.
Long-term follow-up
Participants are monitored for progression-free survival and overall survival.

Trial Overview
The study is testing ABBV-383, a new monoclonal antibody drug, to find the best dose and see how safe and effective it is for treating relapsed or refractory Waldenström macroglobulinemia. Patients will also undergo scans, biopsies, blood tests, and answer questionnaires.
How Is the Trial Designed?

Experimental Treatment
Patients receive ABBV-383 IV as a single push or over 30 minutes to 4 hours on days 1 and 4 of cycle 1 and on day 1 of subsequent cycles. Cycles repeat every 28 days for up to 12 cycles in the absence of disease progression or unacceptable toxicity. Patients also undergo CT or PET/CT during screening and blood and urine sample collection and bone marrow aspiration and biopsy throughout the trial. Additionally, patients with extramedullary disease undergo CT or PET/CT throughout the trial.
