Tiprelestat for Pulmonary Arterial Hypertension
(ATHENA Trial)
What You Need to Know Before You Apply
What is the purpose of this trial?
This trial tests a new treatment called tiprelestat for pulmonary arterial hypertension (PAH), a condition where high blood pressure affects the arteries in the lungs and heart. Researchers aim to determine if adding tiprelestat to standard treatments is safe and improves symptoms compared to a placebo. Participants will receive either different doses of tiprelestat or a placebo for 168 days. This trial suits individuals with PAH who continue to experience symptoms despite current treatments. As a Phase 2 trial, the research focuses on evaluating the treatment's effectiveness in an initial, smaller group of participants.
Do I need to stop my current medications for the trial?
No, you don't need to stop your current medications. In fact, you must continue taking your stable doses of certain medications like PDE5 inhibitors, ERA, sGCS, or prostacyclin analogues, as well as Sotatercept, during the study.
Is there any evidence suggesting that tiprelestat is likely to be safe for humans?
Research has shown that tiprelestat has been tested in five previous studies involving over 100 participants. These studies found that tiprelestat is generally safe and well-tolerated. A six-month study supported this positive safety record, indicating that people taking tiprelestat did not experience serious side effects. Additionally, another trial demonstrated that administering tiprelestat to patients for 7 days was safe and well-tolerated. While researchers continue to test tiprelestat for pulmonary arterial hypertension, these findings provide strong evidence of its safety for humans.12345
Why do researchers think this study treatment might be promising?
Tiprelestat is unique because it offers a new approach to treating pulmonary arterial hypertension (PAH). Unlike current treatments that often focus on widening blood vessels or reducing blood pressure, tiprelestat works by inhibiting neutrophil elastase, an enzyme that can damage lung tissue. This innovative mechanism might protect the lungs from further damage, potentially improving outcomes for patients with PAH. Researchers are excited because this could provide an additional layer of treatment, potentially in combination with existing therapies like endothelin receptor antagonists or phosphodiesterase-5 inhibitors, offering new hope for people living with this challenging condition.
What evidence suggests that tiprelestat might be an effective treatment for pulmonary arterial hypertension?
Research has shown that tiprelestat is under investigation as a potential treatment for pulmonary arterial hypertension (PAH), a condition with limited effective treatments and a 5-year survival rate of only 57%. Earlier studies examined the safety of tiprelestat in over 100 people and reported encouraging safety results. Tiprelestat affects certain pathways involved in PAH, which may help improve heart and lung function. Although researchers are still gathering information on its effectiveness in humans, these early findings suggest it could be a promising option for further study in treating PAH. Participants in this trial will receive either a 5 mg or 10 mg dose of tiprelestat, or a placebo, to further evaluate its potential benefits.12367

Are You a Good Fit for This Trial?
This trial is for adults aged 18-75 with pulmonary arterial hypertension (PAH) in WHO functional class II-IV, who are already on stable standard treatments. Participants must be able to follow study procedures and use reliable contraception if of childbearing potential.Inclusion Criteria

Trial Overview
The study compares tiprelestat (at two different doses) plus standard care versus placebo plus standard care in people with PAH. Patients are randomly assigned to receive either the medication or a placebo alongside their usual treatment.
