Etrasimod for Pouchitis
(ESPIRIT Trial)
What You Need to Know Before You Apply
What is the purpose of this trial?
This trial explores whether etrasimod can prevent or treat pouchitis, a condition that sometimes occurs after surgery for ulcerative colitis. Researchers will compare etrasimod to a placebo to determine its effectiveness in reducing pouchitis symptoms. This study targets individuals who have undergone specific surgery for ulcerative colitis and are at high risk for pouchitis, particularly those with a previous episode that improved after antibiotics. As a Phase 2 trial, the research focuses on assessing the treatment's effectiveness in an initial, smaller group of participants.
Do I need to stop my current medications for the trial?
The trial protocol does not specify if you need to stop taking your current medications. However, you cannot have used probiotics within 3 months, topical rectal 5-ASA or steroids within 2 weeks, or any biologic or small molecule for UC after surgery. It's best to discuss your current medications with the trial team.
Is there any evidence suggesting that etrasimod is likely to be safe for humans?
Research has shown that etrasimod has been tested in people with ulcerative colitis, a condition similar to pouchitis. In these studies, etrasimod was generally well-tolerated, with most participants not experiencing serious problems. Safety data indicate that side effects were similar to those in the placebo group, with headaches and nausea being the most common.
Another study analyzed data from various trials and found that etrasimod had a good safety record, with serious side effects being rare. These findings suggest that etrasimod is likely safe for conditions like pouchitis, providing some confidence about its safety. However, since this trial is in Phase 2, monitoring for any new safety issues remains important.12345Why do researchers think this study treatment might be promising?
Etrasimod is unique because it offers a new approach to treating pouchitis by targeting the S1P receptor, which plays a key role in regulating immune cell movement. Most current treatments, like antibiotics and corticosteroids, focus on reducing inflammation broadly, but Etrasimod specifically modulates the immune response, potentially leading to fewer side effects. Researchers are excited about Etrasimod because it might provide a more targeted and effective treatment option for patients who haven't found success with traditional therapies.
What evidence suggests that etrasimod might be an effective treatment for pouchitis?
Research has shown that etrasimod may help treat conditions like ulcerative colitis, which resembles pouchitis. One study found that nearly half of the patients taking etrasimod experienced a significant reduction in symptoms within 12 weeks. Long-term results from that study also showed that 64% of patients had fewer symptoms and felt better overall. In this trial, participants will receive either etrasimod or a placebo for 48 weeks. These findings suggest that etrasimod might help manage pouchitis by reducing symptoms and improving quality of life.34678

Are You a Good Fit for This Trial?
This trial is for adults (18+) who have had surgery to remove their colon and create an internal pouch due to severe ulcerative colitis. Participants must be in symptom remission, have healthy blood counts, no major heart issues on ECG, a history of chickenpox or vaccination, and be at high risk for developing pouchitis.Inclusion Criteria
