Sonrotoclax + Zanubrutinib for Mantle Cell Lymphoma

This trial explores how two oral medications, sonrotoclax and zanubrutinib, work with standard CAR-T cell therapy to treat mantle cell lymphoma, a type of blood cancer. The goal is to determine if combining these treatments can be more effective than standard therapy alone. People with mantle cell lymphoma that has returned or isn't responding to treatment, and who have tried at least one other therapy, including a Bruton's Tyrosine Kinase inhibitor, might be suitable candidates. As a Phase 2 trial, the research focuses on measuring the treatment's effectiveness in an initial, smaller group, offering a chance to benefit from potentially more effective therapies.

The trial requires participants to switch from their current Bruton's Tyrosine Kinase inhibitor (BTKi) to zanubrutinib if they are already on a BTKi. Additionally, you cannot use moderate or strong CYP3A inducers or inhibitors during the treatment period with sonrotoclax.

Research has shown that the combination of sonrotoclax and zanubrutinib is generally safe for patients. Studies have found no major safety concerns, such as tumor lysis syndrome (when cancer cells break down too quickly) or heart problems like irregular heartbeat. This indicates that the treatment was manageable and did not cause unexpected side effects.

Researchers are excited about Zanubrutinib + Sonrotoclax for treating Mantle Cell Lymphoma because this combination targets cancer cells in a novel way. While most treatments for this condition, like chemotherapy and immunotherapy, aim at broadly attacking cancer cells, Zanubrutinib is a Bruton’s tyrosine kinase (BTK) inhibitor that disrupts signals crucial for cancer cell survival. Sonrotoclax, on the other hand, is designed to target proteins that help cancer cells avoid death. Together, this duo may offer a more precise and effective approach to attacking the cancer compared to the current standard treatments.

Research has shown that combining the drugs sonrotoclax and zanubrutinib, which participants in this trial will receive, may effectively treat mantle cell lymphoma. One study found that 79% of patients with recurring or hard-to-treat cases saw their cancer shrink or disappear, with 66% experiencing complete remission. Another study reported that all patients responded positively to the treatment by week 48 when using standard doses. Sonrotoclax blocks a protein called Bcl-2, which helps stop tumor growth and kills cancer cells.
