What You Need to Know Before You Apply

What is the purpose of this trial?

This trial tests a new method for treating head and neck cancer using a combination of Pembrolizumab, a cancer-fighting drug, and ultrasound technology. The researchers aim to determine if ultrasound can enhance the drug's effectiveness by more directly targeting the cancer. The trial seeks to establish the safety and efficacy of this method for patients. Suitable candidates have HPV-negative head or neck cancer that ultrasound can reach and measure for changes. As a Phase 1 trial, the research focuses on understanding how the treatment works in people, offering participants the opportunity to be among the first to receive this innovative approach.

Do I have to stop taking my current medications for the trial?

The trial protocol does not specify whether you need to stop taking your current medications. However, you cannot participate if you are on certain immunosuppressive medications or investigational agents, or if you have received a live vaccine recently. It's best to discuss your specific medications with the study team.

What prior data suggests that this protocol is safe for head and neck cancer patients?

A previous study found that pembrolizumab was generally well-tolerated by patients with head and neck cancer. It helped shrink tumors in 57% of patients and led to a partial or complete response in about 25%. The treatment aims to improve effectiveness when combined with ultrasound-induced microbubble cavitation. Although this combination is still under investigation, pembrolizumab has been used safely in other contexts. The most common side effects include tiredness, rash, and nausea, which are usually manageable. This trial is in an early phase, so while the safety data appears promising, it remains under close study.12345

Why are researchers excited about this trial's treatments?

Researchers are excited about this treatment for head and neck cancer because it combines pembrolizumab, an immunotherapy drug, with ultrasound-induced microbubble cavitation. Unlike traditional treatments like surgery, radiation, and chemotherapy, this approach uses ultrasound waves to enhance the delivery and effectiveness of pembrolizumab, potentially boosting the immune system's ability to fight cancer cells more effectively. The use of microbubbles is a novel method that may improve drug penetration into tumors, offering a new mechanism of action that could lead to better outcomes for patients.

What evidence suggests that Pembrolizumab plus ultrasound might be an effective treatment for head and neck cancer?

This trial will investigate the combination of pembrolizumab with ultrasound-induced microbubble cavitation for head and neck cancer. Studies have shown that pembrolizumab alone can shrink tumors in head and neck cancer, with 57% of patients experiencing tumor reduction and 24.8% achieving a partial or complete response. Pembrolizumab enhances the immune system's ability to attack cancer cells. In this trial, ultrasound-induced microbubble cavitation aims to improve drug delivery to the tumor, potentially enhancing pembrolizumab's effectiveness. Early research suggests this method could increase the drug's ability to target cancer cells.12346

Are You a Good Fit for This Trial?

This trial is for adults (18+) with relapsed or metastatic head and neck squamous cell carcinoma that is HPV-negative, has a certain biomarker (CPS >1), and can be reached by ultrasound. Participants need to have good organ function, decent overall health, and at least 6 months life expectancy.

Inclusion Criteria

4. Bilirubin less than or equal to 1.5 x the upper limit of normal (except subjects with Gilbert syndrome, who can have total bilirubin \<3 mg/dl);
My head and neck cancer is HPV-negative and has a CPS score above 1.
My tumor can be seen and measured on an ultrasound and can be biopsied.

Treatment

Participants receive 3 cycles of Pembrolizumab plus Definity infusions every 3 weeks, with ultrasound-induced microbubble cavitation directed at the primary tumor site

9 weeks
3 visits (in-person)

Follow-up

Participants are monitored for safety and effectiveness after treatment, including assessment of adverse events and response rates

3 months
Regular follow-up visits

Long-term follow-up

Participants are monitored for overall survival and progression-free survival

What Are the Treatments Tested in This Trial?

Interventions

  • Pembrolizumab
  • Ultrasound-Induced Microbubble Cavitation

Trial Overview

The study tests pembrolizumab (an immunotherapy drug) combined with Definity microbubbles and targeted ultrasound to help the drug reach tumors better. Patients get three treatments about three weeks apart, each followed by focused ultrasound on their tumor.
